Student Registration Form

We might not be able to accommodate your first choice but we will try our best! Please list your top three most available days and times.

Please Initial on the line for all of the following policies. Read each policy carefully before agreeing to them.

School Year Make-up Class Policy

Each student is allowed three make-up classes during the school year from September to May that must be used within those nine months. Make-up classes cannot be rescheduled day-of. Scheduled make-up classes that exceed the three class limit will be charged a $30.00 fee to your account and you will receive a notification

Summer Make-up Class Policy

Each student is allowed four make-up classes during the summer months of June through August. These four make-up classes can only be used within the summer months. Please contact the studio if your child cannot attend make-up classes during the summer months.

Un-enrollment Policy

Lord of the Light requests that you fill out our "Un-enrollment" form 30 days prior to the month of un-enrollment. Students cannot attend any classes, including make-up classes, once they are un-enrolled.

Lost Art Supply Fee

In the case that a student loses their Curriculum Booklet or ShowTime papers, there will be an additional cost to redeliver these supplies. Curriculum Booklets are an additional $17, and each Basic's ShowTime papers will be $20. For this reason, it is important for students to keep their papers and supplies in a designated place. If you have any concerns or questions, please contact our admin.

Punctuality

Online classes will be open to join 10 minutes before scheduled start time and locked 15 minutes after the scheduled start time for security reasons. If you are unable to join class on time, please contact Mrs. Suzie for a make-up class.

Tuition Policy

After the pro-rated first month, tuition will be charged to your credit card on the first day of every month, regardless of holidays or breaks. If you have a new credit card, your card expires, or to update any other payment information please notify us before the upcoming month to avoid a $15 late charge on your account

Class Transfer Policy

Class changes can be done by contacting our admin at (408)499-7596. Please note that we may not be able to change your child's class as requested due to the limited amount of students per class. Your student may be put on a waitlist for a class if requested

Publicity Waiver & Release

As an art studio, we often photograph and video record our students and their artwork for advertising, promotion and other commercial purposes. You may be asked to optionally review and sign an additional waiver granting us this permission for your child. Please click on the link below to view and sign our "Publicity Waiver and Release" form.

Art Supply Waiver

                                                           (print first & Last name) acknowledge that I wish to participate in receiving supplies from Lord of the Light Art Studio and Gallery (LofL). I understand the certain risks inherent in operating art supplies from LofL and fully accept the risks. These risks include, but are not limited to, injury, disease or other threat of physical harm to myself and other. I understand that there may be a great variety of risks not known or reasonably foreseeable. I understand LofL does not provide insurance to cover expenses for damages and injury, and disease caused by supplies recieved from LofL. I fully release and discharge LofL and its affiliates from all liability in connection with my participation in the aforementioned activity.
